Job summary

Agape Community Health Center is seeking an LPN to support patient care and clinic operations. The role involves assisting providers, managing medication and lab processes, and ensuring accurate documentation within the electronic health record.

Qualifications

Responsibilities

• Acts as LPN, provides resource/support to peers and assists uninsured/underinsured patients.
• Conducts opening duties: monitor vaccine/medication fridge temperatures, prep exam rooms, and other duties.
• Maintains clinic supplies and primary responsibility for inventory ordering.
• Handles clinical inboxes and follows up on lab results and prescriptions.
• Administers medications under supervision of provider and prepares meds as needed.
• Performs duties per Florida VFC program policies and clinic procedures.
• Assists with accurate medical intake (vital signs, allergies, history).
• Completes electronic medical record documentation before provider visit.
• Records specimen collection and lab orders, and completes related forms.
• Prepares rooms, maintains cleanliness, and sterilizes instruments.

Job description

The Licensed Practical Nurse is a key member of the clinic team. He/she plays an important role to help ensure a positive patient experience and efficient clinic operations by assisting healthcare providers in the conduct of patient care. This individual serves as the senior clinical staff member for patient care.

SUPERVISORY RESPONSIBILITIES

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Serves as LPN, acting as a resource and support person to peers. Must become proficient with all resources for non-insured/underinsured patients.
• Conducts opening duties for assigned site including: monitoring of immunization refrigerator and medication refrigerator temperature logs, prep of exam rooms, and other duties as specified to meet organization’s needs
• Ensures clinic is stocked with appropriated supplies
• Maintains medical supply inventory and is primary responsible party for ordering supplies
• Maintains clinical inboxes including following up with patients regarding lab results, outstanding orders, and prescription orders
• Administers medications under the supervision of the primary care provider; assist in the preparation of medications for patients as needed
• Maintains medication order and immunization ordering in accordance with the Florida Vaccines for Children (VFC) Program and Agape Family Health policies and procedures
• Assists medical assistant assigned to provider with accurate medical intake including vital signs, height, weight, allergies (drug and food), past medical history, current complaint
• Completing electronic medical record documentation before patient is seen by primary care provider
• Assisting clinical staff in completing examination and procedures as instructed
• Obtaining laboratory samples and complete associated paperwork such as lab slips and other forms when needed
• Performs directed procedures such as EKG, nebulizer treatments, ear irrigation, suture removal, wound dressings, PPD testing, Rapid HIV testing (if credentialed), venipuncture and other directed procedures in accordance with OSHA and CLIA guidelines
• Completes patient examination process by providing check out slips, follow up appointment, and visit summary to patients
• Prepares examination rooms and ensures rooms are stocked with needed supplies; maintains cleanliness of exam rooms
• Cleans and sterilizes instruments and disposes of contaminated supplies
• Assists in the coordination of specialty clinics as required, including maintenance of exam rooms, supplies and clinic records
• Tracks laboratory results and provides monthly numbers for required reports
• Follows universal precautions to protect self and patients
• Participates in all appropriate team meetings and required training sessions. Participates in other clinical or administrative projects as assigned by the Clinic, Director of Quality, or Clinical Operations Managers. Assist with trainings for existing and new employees.
• Completes quality assurance activities on equipment and medical devices as well as testing processes used in the clinic.
• Notifies supervisor immediately if equipment is not functioning properly, supplies are needed, or facility is not operating as expected.
• Manages patient flow and volume. Keeps patients informed of expected wait times
• Responsible for clerical tasks in both the front and back office associated with patient care and proper record keeping. Ensures accuracy in documentation.
• Cross trained to perform Front Office duties including greeting patients, obtaining authorizations, check in/out, communicating wait times, etc.
• Answers telephone as needed

PATIENT CENTERED MEDICAL HOME (PCMH)

• Expected to proactively collaborate and integrate with members of their team, other departments and directorates to achieve optimal care for Patient Centered Medical Home (PCMH) model.
• Actively participates in PCMH Team huddles o Promotes effective communication with staff, patients, families, and between other departments.
• Prepares patient electronic health record and other appropriate forms prior to patient’s appointment
• Utilizes automated programs and information technology tools to facilitate the patient experience.
• Data entry for population health metrics and health promotion
• Assist in maintaining and updating patient registries
• Utilizes the patient’s preferred method of communication to provide integrative and comprehensive care
• Utilizes clinic available tools for proactive patient management of their health care needs. It is expected he/she will participate in updating patient data by utilizing communication and management tools (i.e., Athena computer system).
• Communicates to patients utilizing asynchronous messaging to improve communication and facilitate care through non-traditional means
