Autumnwood Care Center in Tiffin, OH is seeking an Evening Shift Licensed Practical Nurse (LPN) to join our compassionate care team. You will provide skilled nursing care, administer medications, and support residents' physical and emotional needs on the evening shift.

As part of an interdisciplinary team, you will develop individual care plans, monitor conditions, document progress, and collaborate with rehabilitation and social services to ensure high-quality care and regulatory compliance.

Formación

• Valid LPN license in the state of Ohio.
• Experience providing skilled nursing care to residents is preferred.
• Ability to develop and follow individualized care plans.
