Job summary

Heritage Hills Rehabilitation & Healthcare Center in Rhode Island is seeking a part-time Licensed Practical Nurse (LPN) to join our team on the 3-11 shift. Daily pay is available and the pay rate is $31-$40 per hour, all-inclusive.

The LPN will provide patient care, follow care plans, administer medications, monitor residents, document accurately, and support a safe, respectful environment for residents and families within a 100-bed facility.

Qualifications

• Current, active LPN license in Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.
