*Summary*

Currently hiring experienced Licensed Practical Nurses (LPNs) to fill multiple openings with a home care agency located in Raritan, NJ. Interested candidates should have a minimum of one year of documented experience in a home health setting, primary care clinic, or healthcare facility.

Pay rate $30 to $38 per hour, depending on experience and assignment.

*Responsibilities of the Licensed Practical Nurse (LPN)*
* Provide thoughtful care to clients based on assessment and personalized Plan of Care
* Provide compassionate support to clients and their families
* Monitor and track changes in the client's condition and needs, keeping the Director of Nursing and/or any other appropriate personnel informed
* Proactively update the Director of Nursing on any health changes or concerns

*Requirements of the Licensed Practical Nurse (LPN)*
* Current unrestricted LPN license in the New Jersey
* Current BLS certification
* Minimum of one year of documented clinical experience
* Reliable transportation with valid driver's license and automobile insurance
* Strong communication skills and ability to work collaboratively with healthcare teams
* Varied assistance is required, depending on the mobility of the client
