Job summary

Elara Caring seeks an experienced Private Duty Licensed Practical Nurse to contribute to home health care by reviewing plans, assessing patient needs, and coordinating with the care team. The role emphasizes accurate documentation, timely communication, and adapting to changing patient conditions to ensure high-quality care in patients' homes.

Qualified candidates are LPNs licensed in Indiana (or NLC), with 1+ year of clinical nursing experience, and able to lift up to 50 pounds.

Qualifications

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1+ year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and assist up to 50 pounds.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.

Responsibilities

Job Description

As Private Duty Licensed Practical Nurse you’ll contribute to our success in the following ways:

• Review patient care plans, physician orders, and clinical information before providing care.

• Use clinical assessment skills to identify patient needs and make appropriate care decisions.

• Monitor patients for changes in condition and promptly communicate concerns to the physician and care team.

• Collaborate with the RN Case Manager and clinical leadership to determine appropriate interventions when patient needs change.

• Maintain accurate and timely documentation of care provided, patient condition, and changes in the home or caregiver situation.

• Maintain flexibility with scheduling and patient assignments to support changing patient care needs.

What is Required?

• Licensed Practical Nurse (LPN) in the state or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

• 1+ year of experience as a nurse in a clinical setting

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
