Job summary

Elara Caring is hiring a Licensed Vocational / Practical Nurse in Indiana to provide high-quality home health care. You will follow individualized care plans, monitor patient progress, and work with an RN Case Manager to adjust plans as needs evolve.

Role emphasizes communication with physicians, patients, and families to ensure coordinated care. You will document medications, educate patients and caregivers, and strive to prevent avoidable hospitalizations through proactive, compassionate care.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds
• Must possess a valid driver's license or have reliable transportation to travel between patient homes and work locations

Responsibilities

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

• 1 year of experience in a clinical care setting

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Experience in a hospice or home health environment is preferred
