Job summary

Elara Caring is seeking a Private Duty Licensed Practical Nurse to provide in-home care across Indiana. You will review plans, assess patient needs, monitor conditions, and coordinate with the RN Case Manager to adjust care as patient needs change.

The role requires an LPN license in Indiana, 1+ year of clinical nursing experience, ability to lift up to 50 lbs, and reliable transportation to travel between patient homes. Comprehensive benefits are offered.

Qualifications

As Private Duty Licensed Practical Nurse you’ll contribute to our success in the following ways:

• Review patient care plans, physician orders, and clinical information before providing care.

• Use clinical assessment skills to identify patient needs and make appropriate care decisions.

• Monitor patients for changes in condition and promptly communicate concerns to the physician and care team.

• Collaborate with the RN Case Manager and clinical leadership to determine appropriate interventions when patient needs change.

• Maintain accurate and timely documentation of care provided, patient condition, and changes in the home or caregiver situation.

• Maintain flexibility with scheduling and patient assignments to support changing patient care needs.

What is Required?

• Licensed Practical Nurse (LPN) in the state or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

• 1+ year of experience as a nurse in a clinical setting

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
