Job summary

Aviata Health Group is hiring a Licensed Practical Nurse (LPN) for full-time day and night shifts at Aviata at University Hills in Pensacola, FL. You will provide direct patient care under a nurse's supervision in a skilled nursing setting.

Qualified candidates hold an active LPN license; IV certification is preferred. Experience in SNF/Rehab is a plus, and new grads are welcome to apply. We offer strong benefits and growth opportunities.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• IV certification is preferred.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.
• Other duties as assigned.

Major Responsibilities

The Licensed Practical Nurse (LPN) care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.
