Job summary

Aviata Health Group is seeking a compassionate Licensed Practical Nurse (LPN) to join Aviata at the Gardens - Tallahassee, a skilled nursing and rehabilitation center in Tallahassee, FL. The LPN will provide patient-centered care to residents and patients while advancing skills in a supportive environment.

The role requires an active LPN license in Florida, with preference for SNF/Rehab experience. 12-hour night shifts (7p-7a) and a comprehensive benefits package are offered.

Qualifications

• Must possess a current, unencumbered LPN license in Florida.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Must be compassionate and dedicated to high-quality care.

Responsibilities

• Provide direct patient care to residents and patients under supervision of a nurse.
• Assist in implementing individualized treatment plans for each resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls and Pressure Sore Programs.
• Document thorough evaluations of residents’ medical status.
• Perform other duties as assigned.

Job Summary

Aviata Health Group is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Aviata at the Gardens - Tallahassee, a skilled nursing and rehabilitation center in Tallahassee, Florida. As a LPN, you will play a vital role in providing patient-centered care to our residents and patients, while advancing your skills in a dynamic and supportive environment.

Job Responsibilities

• Provide direct patient care to residents and patients under the direction and supervision of a professional nurse
• Assist in the implementation of individualized treatment plans for each assigned resident
• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation
• Oversee Falls Program and Pressure Sore Program
• Provide nursing services, including conducting and documenting thorough evaluations of each resident’s medical status
• Perform other duties as assigned

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• You must be qualified, compassionate, and dedicated to a job well done
