Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility. You will provide total nursing care to residents during your shift, supervise team members, and ensure compliance with facility standards.

Responsibilities include developing care plans, administering medications, educating residents and families, acting as charge nurse when needed, and staying current with facility policies. PM & Overnight shifts available.

Qualifications

• Graduate of an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to deliver empathetic, patient-centered care.
• Ability to work with diverse resident populations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment promoting residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and procedures.
• Monitor team member performance and assist with development and evaluation.
• Delegate and ensure completion of nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and respond to significant changes in resident conditions.
• Document care, responses, and relevant information per guidelines.
• Act as charge nurse: answer questions, resolve issues, support care decisions.
