Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility in Chicago, IL. You will provide total nursing care to residents on your unit and supervise team members to ensure high-quality, compliant care.

This on-site role emphasizes collaboration with the healthcare team and ongoing education to maintain current facility policies and best practices.

Qualifications

• Graduation from an accredited nursing education program.
• Active Licensed Practical Nurse (LPN) license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.
• Must be able to work with diverse resident populations, including elderly and mentally ill individuals.
• Must be physically able to perform nursing duties with or without accommodations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
