Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate care team at our skilled nursing facility in Palos Park, IL. You will provide total nursing care to residents, supervise team members, and ensure compliance with facility standards.

Responsibilities include administering medications, developing care plans with the healthcare team, educating residents and families, and acting as a charge nurse when needed.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Able to follow policies, maintain safety, and deliver empathetic care.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment promoting residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures.
