Aviata at The Palms in Palm Harbor, FL is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct patient care for residents in a health and rehabilitation setting.

Shifts available include FT Day/Night and PT Night; interview on the spot with walk-ins Wednesdays 10a-3p. The team emphasizes compassionate, patient-centered care and supports ongoing education and benefits.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• You must be qualified, compassionate, and dedicated to a job well done.

Responsibilities

• Assist in implementing individualized treatment plans for each resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services to residents.
• Conduct and document a thorough evaluation of each resident’s medical status on admission and during care.
• Other duties as assigned.

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Aviata at The Palms in Palm Harbor, Florida. As an LPN at our health and rehabilitation center, you will provide direct patient care to our residents, assisting them in activities of daily living under the direction and supervision of a professional nurse.

Responsibilities:

• Assist in the implementation of individualized treatment plans for each assigned resident
• Supervise CNAs and participate in their evaluation

• Provide nursing services
