Job summary

Interim HealthCare in Portsmouth, Ohio is seeking a Licensed Practical Nurse (LPN) to provide 1:1 patient care in the school system. This school-based assignment offers Monday–Friday hours aligned with school days and a local setting in Scioto County.

The LPN will work with a team including RNs and aides to monitor students, take vitals, document care, and educate families about the plan of care and safety.

Qualifications

• Graduate of an accredited practical nursing program and active Ohio LPN license.
• Minimum of 1 year of LPN experience.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills and compassion.

Responsibilities

• Provide care to patients under supervision of an RN.
• Collaborate with physicians, RNs, CNAs and aides to deliver care.
• Take vitals, assess, document progress and report changes to RN.
• Assist RN with plan of care and teach self-care.

Interim HealthCare is looking for a dedicated Licensed Practical Nurse (LPN) to provide 1:1 patient care in the school system.

Schedule: Monday–Friday (school hours)
Location: Local school system (Scioto County)

As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

• Provide care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
