Job summary

AA Healthcare Management in Illinois is seeking a Licensed Practical Nurse (LPN) to provide quality resident care, administer medications as ordered, and supervise day-to-day nursing tasks performed by CNAs in our facility.

You will coordinate care across disciplines, document resident information, participate in care conferences, support admission and discharge processes, and engage in ongoing education to maintain safe, compliant, and compassionate care.

Qualifications

• Must hold an active Illinois LPN license.
• Experience in long-term care is a plus.
• Ability to coordinate care with CNAs and physicians.

Responsibilities

• Prepare and administer medication as ordered by the physician.
• Review medication orders for completeness and accuracy.
• Transcribe physician orders.
• Document resident information and notify physician/family as needed.
• Assist in the Admission/Discharge process.
• Complete accident/incident reports and follow-up.
• Participate in care conferences and update care plans as needed.
• Maintain good rapport with all disciplines to meet resident needs.
• Interact tactfully with residents, families, and staff.
• Participate in continuing education programs.

Licensed Practical Nurse (LPN) Job Overview

The primary purpose of the Licensed Practical Nurse (LPN) is to provide quality nursing care to our residents and to supervise the day-to-day nursing functions performed by the Certified Nursing Assistants. The Licensed Practical Nurse (LPN) is also responsible for coordinating all aspects of a resident’s care with other disciplines in the facility. Job duties are governed by the current Federal and State regulations that govern the facility.

Licensed Practical Nurse (LPN) Job Duties

• Prepare and administer medication as ordered by the physician.


• Review medication orders for completeness of information and accuracy.


• Transcribe physician orders.


• Document all resident information as necessary, notifying physician and family as needed.


• Assist in the Admission/Discharge process.


• Complete accident/incident reports, follow up investigation process.


• Be familiar with the care planning process and participate in care conferences, updating as needed.


• Develop and maintain a good working rapport with all disciplines to ensure that all aspects of resident needs can be adequately maintained.


• Should be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.


• Be willing to participate in continuing education programs.
