Job summary

Northbrook Health and Rehab is seeking a Licensed Practical Nurse (LPN) to provide high‑quality nursing care to residents and coordinate day‑to‑day nursing functions with CNAs. The LPN will ensure care is aligned with current Federal and State regulations governing the facility.

The role requires an active Illinois LPN license and involves collaboration with physicians and families to coordinate all aspects of resident care. Strong communication and supervision skills are essential.

Qualifications

• Must hold current Illinois LPN license.
• Experience in long‑term care is a plus.
• Ability to supervise Certified Nursing Assistants and coordinate care.

Responsibilities

• Prepare and administer medications as ordered by the physician.
• Review medication orders for completeness and accuracy.
• Transcribe physician orders.
• Document resident information and notify physician and family as needed.
• Assist in Admission/Discharge processes.
• Complete accident/incident reports and follow‑up investigations.
• Participate in care planning conferences and update as needed.
• Maintain good rapport with all disciplines to meet resident needs.
• Deal tactfully with staff, residents, families, visitors and agencies.

Licensed Practical Nurse (LPN) Job Overview

The primary purpose of the Licensed Practical Nurse (LPN) is to provide quality nursing care to our residents and to supervise the day-to-day nursing functions performed by the Certified Nursing Assistants. The Licensed Practical Nurse (LPN) is also responsible for coordinating all aspects of a resident’s care with other disciplines in the facility. Job duties are governed by the current Federal and State regulations that govern the facility.

Licensed Practical Nurse (LPN) Job Duties
