Job summary

Alternate Solutions Home Health of Columbus is seeking an LPN to provide skilled nursing care in patients' homes within assigned geographic areas. You will follow physician orders, monitor conditions, and support patients and families in planning care.

The role requires current LPN licensure, 1+ year acute care experience (2+ preferred), and excellent communication. Travel is required; on-call may be part of duties. Equal opportunity employer.

Qualifications

  • Licensed Practical Nurse with current license in the state of employment.
  • Minimum of one year of experience as an LPN in an acute care setting; two years preferred.
  • Home care experience is desired.
  • Ability to communicate effectively with patients, families, physicians and coworkers.

Responsibilities

  • Provides skilled nursing care in a patient's home as ordered by the attending physician.
  • Performs care following order sets and goals made by RN.
  • Observes and monitors patient conditions.
  • Notifies RN/CM of changes in patient condition.
  • Administers prescribed medications.
  • Counsels patient and family on disease/injury processes.
  • Incorporates patient and family in plan of care development.
  • Helps decrease re-hospitalizations by teaching preventative measures.
  • Maintains Medicare/Federal documentation compliance.
  • Documents all treatment, assessments, and education.
  • Maintains active LPN license in state of employment.
  • Communicates changes to scheduler.
  • Participates in on-call requirements and case conferences.
  • Attends trainings and agency meetings.
  • Maintains professional appearance as company representative.
  • Follows agency policies and procedures.
  • Other duties as assigned.

QUALIFICATIONS & ATTRIBUTES:
  • Licensed Practical Nurse with current license in the state of employment.

  • Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred.

MAJOR AREAS OF RESPONSIBILITY:
  • Provides skilled nursing care in a patient’s home as ordered by the attending physician.

  • Performs care by following order sets and goals made by RN.

  • Observes and monitors patient conditions.

  • Notifies RNCM of any changes in patient condition.

  • Administers medication as prescribed by the physician.

  • Counsels patient and family on the disease/injury processes and how to manage.

  • Incorporates patient and family in development of plan of care.

  • Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.

  • Listens to patient and family members.

  • Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.

  • Documents all aspects of treatment, assessments, and patient education.

  • Maintains active LPN license in state of employment.

  • Communicates with scheduler any changes outside the normal.

  • Participates in all on-call requirements and case conferences.

  • Attends in-service trainings and mandatory agency meetings.

  • Maintains a professional appearance as a representative of the company.

  • Understands and practices agency policies and procedures.

  • Other duties as assigned.

ESSENTIAL FUNCTIONS:
  • Drives to patient’s primary location per scheduled visit.

  • Daily attendance at assigned visit locations.

  • Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines.

  • Completes and submits all required documentation within specified company requirements.

  • Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.

  • Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
