Job summary

Alternate Solutions Health Network seeks a Licensed Practical Nurse to provide skilled home health care under physician orders. You will observe patient conditions, administer medications, and educate families on disease processes and care plans.

The ideal candidate has 1–2 years of LPN experience in acute care and home health, with strong communication and a compassionate, patient-centric approach. Valid driver’s license required; occasional on-call duties apply.

Qualifications

• Licensed Practical Nurse with current license in the state of employment.
• Minimum of one year of LPN experience in acute care; two years preferred.
• Home care experience is desired.
• Effective communication with patients, families, physicians and co-workers.
• Calm, patient, accommodating and compassionate demeanor.
• Knowledgeable on nursing best practices.
• Ability to make appropriate nursing judgments and prioritize tasks.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the physician.
• Observes and monitors patient conditions and notifies RNCM of changes.
• Administers medication as prescribed by the physician.
• Counsels patient and family on disease management and plan of care.
• Documents treatment and education per Medicare guidelines.
• Maintains active LPN license and participates in on-call requirements.

Major Areas Of Responsibility

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Performs care by following order sets and goals made by RN.
• Observes and monitors patient conditions.
• Notifies RNCM of any changes in patient condition.
• Administers medication as prescribed by the physician.
• Counsels patient and family on the disease/injury processes and how to manage.
• Incorporates patient and family in development of plan of care.
• Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
• Listens to patient and family members.
• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
• Documents all aspects of treatment, assessments, and patient education.
• Maintains active LPN license in state of employment.
• Communicates with scheduler any changes outside the normal.
• Participates in all on-call requirements and case conferences.
• Attends in-service trainings and mandatory agency meetings.
• Maintains a professional appearance as a representative of the company.
• Understands and practices agency policies and procedures.
• Other duties as assigned.

Essential Functions

• Drives to patient’s primary location per scheduled visit.
• Daily attendance at assigned visit locations.
• Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines.
• Completes and submits all required documentation within specified company requirements.
• Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.
• Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
