• Flexible schedule as needed.
• Computer literacy including Microsoft Office Suite and EMR.
• Ability to explain physician’s instructions and multi-task.
• Experience with diverse populations.

Responsibilities

• Assists with patient care including checking vital signs, blood pressure, pulse rate, breathing, temperature, and weight, discussing chief health concerns, administering injections, and performing tests.
• Takes patient’s medical history, pretreatment formalities, and enters data in the computer system.
• Follows orders of a physician and registered nurse in providing care.
• Documents all care given to patients in thorough detail.
• Handles proper charting, documentation, and maintenance of patient's medical records.
• Check, clean, and dresses wounds.
• Reinforces instructions given to patients by RN or Physician.
• Notifies RN or Physician of significant findings or changes in patient’s physical or mental condition.

The Role

Under the general supervision of the RN and/or Physician, the Licensed Practical Nurse-LPN will support providers in the provision of care including recording vital signs, discussing chief health concerns, performing testing, and assisting with procedures and documentation. Who You Will Be

• Someone with at least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI).
• Available to work a flexible schedule as needed in response to participant and staff needs.
• Computer literacy including proficiency in Microsoft Office Suite and EMR.
• Ability to put patients at ease and explain physician’s instructions.
• Ability to multi-task with excellent organizational skills.
• Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently.
