The Role

Under the general direction of the RN and/or Physician, the Licensed Practical Nurse (LPN) will support providers in the provision of care, including recording vital signs, discussing chief health concerns, performing testing, assisting with procedures, and documentation. The incumbent will assess, plan, evaluate, and deliver nursing care in collaboration with RN and/or Physician, supporting the mission and goals of Samaritan Daytop Village.

Responsibilities
  • Assist with patient care including checking vital signs, blood pressure, pulse rate, breathing, temperature, weight, discussing chief health concerns, administering injections, and performing tests.
  • Take patient’s medical history, complete pretreatment formalities, and enter data into the computer system.
  • Follow orders of physician and registered nurse in providing care.
  • Record detailed reports of care given in patients’ records.
  • Handle proper charting, documentation, and maintenance of patients’ medical records.
  • Check, clean, and dress wounds.
  • Reinforce instructions given to patients by RN or Physician.
  • Notify RN or Physician of significant findings or changes in patient’s physical or mental condition.
  • Place phone calls to pharmacies to order prescriptions.
  • Maintain confidentiality of patient records.
  • Pull and prepare patient charts.
  • Maintain neatness of work area.
  • Stock patient rooms with supplies.
  • Provide emotional support as needed.
  • Initiate emergency assistance as per policies and procedures.
  • Attend staff development trainings and nursing conferences.
  • Keep skills current and increase knowledge and productivity.
  • Perform other duties as requested.
Qualifications
  • NYS LPN licensure.
  • BLS certification.
  • High School diploma or GED.
  • Graduate of a formal Technical or Vocational LPN Program.
  • At least one (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI).
  • Available to work a flexible schedule as needed.
  • Computer literacy including proficiency in Microsoft Office Suite and EHR.
  • Ability to put patients at ease and explain physician’s instructions.
  • Ability to multi-task with excellent organizational skills.
  • Experience working with diverse racial, ethnic, and socioeconomic backgrounds.
  • Knowledge of medical terminology.
  • Team player with the ability to work independently.
