Job summary

Personal Healthcare Management LLC is seeking an LPN to join our team in New York, NY. The role offers competitive starting pay with rates from $31.25 to $34.38 per hour, depending on experience, with day, evening, and overnight shifts available.

You will provide safe resident care, administer medications, and document care plans according to standards. A valid LPN license and at least 1 year of experience are required.

Qualifications

• Must hold an active LPN license and 1 year of clinical experience.
• Provides safe resident care following established standards.
• Will adhere to medication administration standards.
• Shift coverage includes day, evening and overnight shifts.

Responsibilities

• Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures.
• Participates in the improvement process of quality resident care.
• Incorporates age-specific needs and treatments into care.
• Communicates resident information and plan of care during handoffs and shift reports.
• Administers medications according to the five rights and documents accordingly.
• Performs treatments using sterile or clean technique.
• Administers external feedings as prescribed.
• Completes care-related activities within shift time and cooperates with reassignments.
• Reports care problems and proposes solutions to nursing management.

Shift Hours Available:6am-2pm, 2pm-10pm and 10pm-6am.

Functions

• Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Maintains awareness of new updates standards and incorporates them into his/her nursing practice through out the continuum of care.
• Participates in the improvement process of quality resident care.
• Incorporates age-specific needs, provision of care, procedures, and treatments.
• Communicates pertinent resident information and plan of care before breaks, during transfer, and at shift report.
• Accurately administers medications according to the five rights of medication administration and documentation of same.
• Accurately performs treatments utilizing sterile or clean technique.
• Accurately administers external feedings.
• Completes resident care-related activities within scheduled shift time periods.
• Cooperates with reassignments made in the interest of resident care.
• Reports resident care problems and possible solutions to the appropriate nursing management personnel.
• Delegates resident care problems and possible solutions to the appropriate nursing management personnel.

Working Environment

Safety Equipment:Job may require the use of safety equipment, such as HEPA mask, for infection prevention.

Bloodborne Pathogens Exposure:OSHA Category I- Routine/anticipated exposure. Work involved exposure to blood, body fluids, or tissue.
