Job summary

Yardley Rehabilitation and Healthcare Center is seeking a licensed practical nurse (LPN) to join a compassionate team in Yardley, PA. This full-time or part-time role offers daily pay and an opportunity to provide patient-centered care in a sub-acute and long-term facility.

You will administer medications, follow care plans, and collaborate with an interdisciplinary team to support residents’ physical and emotional needs in a home-like setting.

Qualifications

• Current, unencumbered LPN license in Pennsylvania.
• BLS/CPR certification required.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care according to nursing standards and the written plan of care.
• Follow and implement individualized care plans.
• Administer medications and treatments per plan and facility policies.
• Monitor residents, report changes to supervisor/physician promptly.
• Document accurately and timely per orders and standards.
• Support a safe, respectful environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Pennsylvania.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
