Job summary

Home Health of Western Colorado in Montrose, CO is seeking an LPN to provide compassionate nursing care in patients

’s homes, with 8-hour shifts, Monday through Friday and occasional weekends. This role offers competitive pay and meaningful, purpose-driven work within a VOANs home health program serving the Montrose area.

Qualifications

  • Graduate from an accredited nursing program with Colorado LPN licensure.
  • Ability to read charts, orders, and plans of care.
  • Must drive to client homes in own vehicle with valid license and insurance.
  • Experience with computer documentation and timekeeping.

Responsibilities

  • Administer prescribed medications and treatments per orders.
  • Observe and report client conditions to RN.
  • Provide nursing care under supervision.
  • Maintain accurate documentation per agency policies.
  • Coordinate with inter-disciplinary team.

Pay Range: $29 - $35.25 per hour based on previous work experience.
Schedule: Fulltime Monday - Friday, 8-hour shifts, occasional weekends

What You Bring (Requirements)

  • Graduate from an accredited school of nursing with current licensure as a Licensed Practical Nurse in the state of Colorado.
  • Ability to understand clients’ charts, EHR Practitioner’s orders, clients’ plan of care, medication orders.
  • Will be expected to drive to client’s home in own vehicle, maintain insurance and have a valid driver’s license.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

What You’ll Be Responsible For

The Licensed Practical Nurse (LPN) shall perform in accordance with state and local laws and within the guidelines of his/her professional organization. All functions shall be performed in accordance with the established policies and practices of Agency and the particular institution in which the LPN is performing his/her duties. The LPN is qualified by education, training, experience and demonstrated abilities to give nursing care under the direction of a RN and to have the knowledge of the principles of psychological, biological and social sciences.

  • Role models positive communication skills with RNs, LPNs, C.N.A.s, PTs, Ots, PTAs, COTAs, inter-disciplinary team, customers, clients and client representatives.
  • Observes, records, and reports to supervisor regarding clients’ conditions, reaction to drugs and treatments and significant events.
  • Perform nursing techniques for the comfort and wellbeing of the client.
  • Administers prescribed medications and treatments per practitioner’s orders.
  • May administer professional services, such as catheterization, tube feedings, suctioning, applying/changing bandages, packs, colostomies and drainage bags. Provide range of motion exercises, care of
    the dying/expired client, etc.
  • Maintains all required documentation in an accurate and timely manner according to policies and procedures of the agency, including documentation of medications and treatments.
  • Other duties as assigned.
