Job summary

CareCore at Heritage in Minster, Ohio is seeking a dedicated Licensed Practical Nurse (LPN) to join our growing long-term care team. This full-time role offers evening hours: 6:00pm–6:00am with every 3rd weekend rotation.

You will provide direct nursing care per established care plans, administer medications and treatments, monitor residents, and communicate changes to physicians and the interdisciplinary team. You will document care accurately and support STNAs/CNAs as needed.

Qualifications

• Active Ohio LPN license in good standing.
• Experience with PointClickCare preferred but not required.
• Long-term care experience preferred, but not required.
• Strong clinical and communication skills.
• Compassionate, dependable, and team-oriented.
• Ability to work effectively in a fast-paced environment.

Available Schedules:
