Job summary

CareCore at Heritage in Minster, Ohio is seeking a dedicated Licensed Practical Nurse (LPN) to join our growing team in a long-term care setting. The role offers full-time schedules with day and night shifts and every-third-weekend rotation.

You will provide direct nursing care, administer medications, monitor residents, document care, and support STNAs/CNAs, all while promoting a resident-centered environment and collaborating with a multidisciplinary team.

Qualifications

• Current Ohio LPN license in good standing.
• Experience with PointClickCare is preferred but not required.
• Long-term care experience preferred, but not required.
• Strong clinical and communication skills.
• Compassionate, dependable, and team-oriented.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans.
• Administer medications and treatments as ordered.
• Monitor resident conditions and communicate changes to physicians and the interdisciplinary team.
• Accurately document care and services provided.

CareCore at Heritage is seeking a dedicated and dependable Licensed Practical Nurse (LPN) to join our growing team in Minster, Ohio. Available Schedules

• 6:00am-6:00pm full time, every 3rd weekend rotation.
• 6:00pm-6:00am full time, every 3rd weekend rotation.
