Job summary

Crestwood Center is seeking Licensed Practical Nurses (LPNs) for full-time evening and night shifts. Part-time and per diem roles are available across all shifts, with 8- and 12-hour options. A $5,000 sign-on bonus is offered for full-time employment, along with comprehensive benefits and enhanced pay in lieu of benefits.

As an LPN, you will support RN-driven care, participate in assessments, administer medications, and supervise unlicensed staff to ensure quality outcomes for residents.

Qualifications

• Graduate of an approved practical or vocational nursing program.
• Current license as LPN with state Board of Nursing (or pending licensure).
• Maintain BLS/CPR certification as required by practice setting.

Responsibilities

• Provide efficient and effective nursing care under RN supervision.
• Evaluate patients/residents and monitor changes in condition.
• Administer medications and treatments per physician orders.
• Supervise and train unlicensed assistive personnel.

Responsibilities

Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction.

• Evaluate patients/residents to collect data, observe condition and report changes in condition.
• Contribute to nursing assessments and care planning.
• Administer medications and performs treatments per physician orders.
• Supervise and monitor patient care provided by unlicensed staff.

Qualifications

• Must be a graduate of an approved school for practical or vocational nursing.
• Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
• Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
