Job summary

Cherokee Park Rehabilitation in Louisville, KY is hiring an LPN for full-time day shifts. You will provide bedside care, monitor vital signs, administer medications, and coordinate with RNs and physicians to support resident well-being.

Qualifications

• Current LPN license in the state of Kentucky.
• At least one year of LPN experience in long-term care is preferred.
• Ability to provide bedside care and maintain patient confidentiality.

Responsibilities

• Provide bedside care to patients including personal hygiene maintenance.
• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate).
• Relay information between patients, registered nurses, and physicians.
• Give oral medications and injections to patients.
• Test a patient’s blood sugar levels.
• Urinary catheter insertion and care.
• Wound care and dressing change.
• Assist patients with personal hygiene.

LPN Job Description:

• Provide bedside care to patients including personal hygiene maintenance

• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)

• Collect patient’s health information

• Relay information between patients, registered nurses, and physicians

• Give oral medications and injections to patients

• Test a patient’s blood sugar levels

• Urinary catheter insertion and care

• Wound care and dressing change

• Assist patients with personal hygiene

• Monitor for changes in patients

• Proper charting, documentation, and maintenance of patient’s medical records

• Asks questions when in doubt for clarification by the Registered Nurse.

• Acts as a role model for ancillary personnel.

• Maintains the confidentiality of the patient’s clinical records.

• Exceptional staff training and customer service skills

• Compassionate and friendly demeanor

LPN Requirements:

• Current LPN license in the state

• At least one year of LPN experience in a long-term care setting is preferred.
