Job summary

Journey in Louisville, KY is seeking a registered nurse to join our care team at St. Matthews Transitional Care. The 12-hour shifts rotate 7a-7p and 7p-7a, delivering compassionate, resident-centered care in a community-driven facility.

• Assesses changes in resident status and notifies physician and family.
• Transcribes physician orders to medical records and carries out orders.

Hours

This position follows a7a-7p and 7p-7ashift schedule.

Major Duties and Responsibilities

• Directs the daily activities of the licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in resident's status, notifying the physician and resident's family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.

Required Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Registered Nurse (RN) in practicing state.
• Current CPR certification.
