Job summary

Ascend Hospice & Palliative Care MO is hiring a Nurse Practitioner in Lee's Summit, Missouri for a per diem opportunity. You will conduct visits to patients receiving palliative care and collaborate with the interdisciplinary team to support the patient’s plan of care.

The role requires Missouri NP licensure, a Master’s degree in Nursing, 2–4 years of hospice experience, and willingness to travel. A valid driver's license and reliable transportation are essential to this position.

Qualifications

• Must have a Nurse Practitioner's license in Missouri.
• 2–4 years experience in hospice or related field.
• Palliative care experience preferred.
• Must have a valid driver's license and a reliable source of transportation.
• Must be willing to travel.

Responsibilities

• Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.
