Diamond Ridge Health and Rehabilitation in Lecanto, FL, is seeking a licensed Licensed Practical Nurse (LPN) to join our caring team. The role involves admissions, medication management, and coordinating with the interdisciplinary team to provide high-quality resident care.

Formación

• LPN license is required by the state or a multistate license.
• CPR certification is required.
• Two years of nursing experience preferred.

Responsabilidades

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control procedures and center policies.
• Collaborate with an interdisciplinary team to achieve resident outcomes.
• Identify and report abnormal changes in resident condition per center policy.
• Work under supervision according to state Nurse Practice Act and facility procedures.
• Deliver nursing care for residents requiring long-term or rehabilitative care.
• Collect patient data, observe and report pertinent information.
• Implement and evaluate the patient plan of care per state regulations.
• Direct and supervise care by other nursing staff when needed.
• Maintain documentation requirements and equipment knowledge.
• Maintain confidentiality and residents’ rights.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
• CPR Certification required
• Two Years prior nursing experience preferred
