Job summary

West State Street is seeking nurses to join our per diem pool in New Hampshire. Under RN direction, the Licensed Practical Nurse provides efficient nursing care and supports patient outcomes.

Responsibilities include data collection, assessments, medication administration, and supervising unlicensed staff to ensure quality care for residents seeking flexible, on-call shifts.

Qualifications

• Graduate of an approved practical nursing program.
• Current LPN licensure by the State Board of Nursing or pending licensure.
• Maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice.

• Evaluate patients/residents to collect data, observe condition and report changes in condition.
• Contribute to nursing assessments and care planning.
• Administer medications and perform treatments per physician orders.
• Supervise and monitor patient care provided by unlicensed staff.
