Morgan Rehabilitation and Healthcare Center is hiring a Licensed Practical Nurse (LPN) for part-time shifts (3PM-11PM and 11PM-7AM). The role emphasizes compassionate, high-quality care across a 120-bed Sub-Acute and Long-Term Care setting.

The LPN will support residents’ physical and emotional needs, follow care plans, administer medications, and maintain thorough documentation. A Rhode Island LPN license and BLS CPR are required, with a focus on teamwork and quality care.

Qualifications

• Current, active LPN license in Rhode Island.
• BLS CPR required.
• Ability to care for multiple patients in a fast-paced healthcare environment.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow individualized care plans developed by Nurse Manager and team.
• Administer medications and treatments per plan and facility policy.
• Monitor residents and report changes to supervising nurse/physician promptly.
• Maintain accurate documentation in line with orders and regulatory standards.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
