Job summary

Riverside Post Acute is seeking a Licensed Practical Nurse (LPN) to join our compassionate team in caring for residents in a 240-bed Sub-Acute and Long-Term Care facility. The role emphasizes accurate medication administration, adherence to care plans, and vigilant monitoring of resident needs, with a focus on dignity and well-being.

We offer competitive pay of $28–$31 per hour (all-inclusive) for FT/PT shifts across day, evening, and overnight schedules, including weekends.

Qualifications

• Current, active LPN license in Florida.
• Valid BLS CPR certification.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and clear communication.

Responsibilities

• Provide patient care per standards and written plan of care.
• Follow care plans by Nurse Manager and team.
• Administer medications per plan and policies.
• Monitor residents and report changes promptly to supervising staff.
• Maintain timely, accurate documentation per orders and regulatory standards.
• Support a safe, respectful, therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Florida.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
