Riverside Post Acute, a 240-bed Sub-Acute and Long-Term Care facility in Jacksonville, FL, is hiring an LPN. Join a compassionate team delivering patient-centered care under the guidance of our Nurse Manager and interdisciplinary staff.

We offer daily pay, competitive hourly rates $28-$31, and a supportive workplace with tuition reimbursement and benefits. Current Florida LPN license required, BLS, and ability to handle multiple patients in a fast-paced setting.

Qualifications

• Current, unencumbered active LPN license in Florida.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow individualized care plans set by Nurse Manager and team.
• Administer medications and treatments per policy.
• Monitor residents and report changes promptly to supervisors.
• Maintain accurate documentation per physician orders and standards.

FT/PT: 7 AM - 7 PM

FT/PT: 7 PM - 7 AM

FT: 11 PM - 7 AM Monday - Friday

PT: 7 PM - 7 AM Every Weekend

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Florida.

• Valid BLS Certification (CPR) required.

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
