Woodland Grove Health and Rehab is seeking an LPN for the evening shift to deliver high-quality, patient-centered care to residents in our Florida facility.
You will administer medications, monitor vital signs, assist with ADLs, and support individualized care plans while collaborating with the interdisciplinary team and educating residents and families as needed.

As a Licensed Practical Nurse (LPN), you will play a vital role in delivering exceptional care to our residents. We are seeking a dedicated and compassionate LPN to join our evening shift team.
As an LPN at Woodland Grove Health and Rehab, you will be responsible for providing high-quality, patient-centered care to our residents. Your duties will include:
Administering medications and treatments as prescribed by physicians
Monitoring vital signs and reporting changes to medical staff
Developing and implementing individualized care plans in collaboration with the interdisciplinary team
Providing support with activities of daily living (ADLs) such as bathing, dressing, and toileting
Educating residents and families on care and treatment plans
Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with ADLs
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Maintain accurate and detailed records of resident care and treatment
Participate in quality improvement initiatives to ensure excellence in patient care
Maintain a clean and organized environment, adhering to infection control and safety standards

Current LPN licensure in the state of Florida
