Your Role and Responsibilities
  • Deliver Direct Patient Care: Administer nursing care to hospice patients in accordance with their individualized care plans, focusing on comfort and symptom management.
  • Collaborate with the Care Team: Work closely with registered nurses, physicians, and other healthcare professionals to evaluate and adjust care plans, ensuring comprehensive support for patients.
  • Educate and Support Families: Provide guidance to patients' families on care techniques, disease progression, and available resources, fostering a supportive environment.
  • Maintain Accurate Documentation: Ensure all patient interactions, treatments, and observations are meticulously recorded in compliance with hospice, state, and federal guidelines.
  • Participate in Continuous Improvement: Engage in team meetings and quality assessment activities to enhance the standard of care provided.
What You Need
  • Current LPN license to practice in NJ
