Agape Care Group is seeking a Licensed Practical Nurse (LPN) for hospice care in Alabama. You will work under the supervision of an RN to visit patients and provide routine care, while communicating with the care team and families.

Qualifications

• Graduate of an accredited school of nursing with a current state license as a licensed practical nurse.
• 2+ years of nursing experience in an acute care setting; hospice experience preferred.
• Reliable transportation and ability to lift 80-100 lbs.

Responsibilities

• Conduct patient visits and provide routine nursing care under RN supervision.
• Communicate with the care team, patient and family to coordinate care.
• Support hospice care with compassionate communication and documentation.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
• Experience: 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively.
