Qualifications

  • Graduate of an accredited school of Nursing with NH license or compact license (NLC).
  • Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
  • Valid driver's license and reliable transportation.

Responsibilities

  • Provide skilled, age‑appropriate nursing care to hospice patients in homes and facilities.
  • Educate patients and families about hospice services and care expectations.
  • Develop and update individualized care plans with the interdisciplinary team.
  • Perform comprehensive patient assessments and ongoing reassessments.
  • Deliver treatments and nursing interventions and monitor progress.
  • Support patient and caregiver education to promote comfort and safety.
  • Collaborate with physicians, nurses, social workers, and volunteers for quality care.
  • Attend team meetings and contribute to quality improvement initiatives.
  • Complete agency trainings and pursue ongoing professional development.

What You’ll Do
  • Provide skilled, age‑appropriate nursing care to hospice patients in homes and residential care facilities in accordance with New Hampshire Board of Nursing standards and Hospice best practices.
  • Educate patients, families, and caregivers about the Hospice philosophy, available services, care expectations, and bereavement support.
  • Develop, implement, and update individualized plans of care in collaboration with physicians, patients, caregivers, and the interdisciplinary hospice team.
  • Perform comprehensive patient assessments and ongoing reassessments, documenting care accurately and thoroughly.
  • Deliver treatments and nursing interventions while monitoring patient progress and adjusting care plans as needed.
  • Empower patients and caregivers through education that promotes comfort, safety, and confidence in care.
  • Collaborate closely with physicians, nurses, social workers, chaplains, volunteers, and other healthcare professionals to ensure coordinated, high‑quality care.
  • Participate in interdisciplinary team meetings, patient care conferences, quality improvement initiatives, and agency committees.
  • Attend agency in‑service trainings and continuing education opportunities to support professional growth.
