Job summary

CenClear in Huntingdon, PA is seeking a compassionate Licensed Practical Nurse (LPN) to join our Behavioral Health team. This full-time position supports our mission by delivering high-quality psychiatric and wellness care to clients in our programs.

You will work four 10-hour days, travel within a one-hour radius as needed, and collaborate with prescribers to collect labs, vitals, and documentation while maintaining confidentiality and professionalism.

Qualifications

  • LPN license required and valid in PA.
  • Two years' experience in mental health and substance use services preferred.
  • Ability to travel and work varied hours.
  • Strong attention to detail and accurate documentation.
  • Clearances required prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate

Responsibilities

  • Work 40 hours per week with a four 10-hour days schedule.
  • Assist psychiatric prescribers by collecting labs, vitals, and documentation.
  • Maintain client history and confidentiality.
  • Complete paperwork accurately and timely.
  • Provide wellness coaching and staff training.
  • Collaborate with healthcare providers for continuity of care.

About the Role:

CenClear's Behavioral Health Division is seeking a compassionate and committed Licensed Practical Nurse (LPN) to join our team. This is a rewarding opportunity to support the health and well-being of individuals in our care and make a lasting difference in their lives.

What You'll Do:

  • Work 40 hours per week, 52 weeks per year
  • Current schedule is four 10-hour days with Mondays off (subject to change based on program needs)
  • Occasional travel required for coverage within a one-hour proximity.
  • Assist psychiatric prescribers in providing treatment to clients, collecting labs, reports, information, and vitals for provider
  • Follow up with clients per prescriber request
  • Willingness to learn and adapt to new information and skills and engage in ongoing education, training, and meetings.
  • Self-motivation and ability to work independently as well as a team.
  • Empathy and understanding in dealing with clients and families.
  • Update client history and changes at each appointment.
  • Maintain professionalism and confidentiality
  • Complete paperwork accurately and in a timely manner.
  • Attend and participate in monthly supervisions.
  • Participate in all required CenClear, CCBHC and wellness/health related education and trainings
  • Provide wellness coaching and related training to staff, individuals in services and community partners
  • Promote a culture of health and wellness through ongoing participation in meetings and community outreach
  • Collaborate and coordinate with healthcare providers for continuity of care
  • Facilitate referrals, as needed
  • Collect required data for outcomes measurement and contract requirements and document appropriately
  • Submit all necessary administrative paperwork according to procedures
  • Document all wellness coaching sessions and other wellness/integration activities
