Job summary

Bristol Hospice in Hood River, OR is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. This full-time, hourly position supports end-of-life care and works Monday–Friday, 8am–5pm within a 60-mile service radius of Hood River.

Must hold an Oregon LPN/LVN license with at least 3 years of nursing experience (hospice or medical/surgical preferred). We offer competitive benefits and a mission-driven culture focused on compassionate care.

Qualifications

• Must be licensed as LPN/LVN in Oregon.
• Minimum 3 years nursing experience; hospice or medical/surgical preferred.
• Ability to educate patients and families about care plans.

Responsibilities

• Provide direct patient care under State Nurse Practice Act and hospice protocols.
• Document patient services accurately and promptly.
• Educate patients and families on diagnosis and self-care.
• Coordinate with interdisciplinary team for hospice services.
• Manage pain and symptoms and adjust care as needed.
• Support continuous professional growth and QA activities.
• Perform other duties as assigned by RN.

QualificationsAn Average Day:

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
