Job summary

Elara Caring is seeking a Licensed Vocational/Practical Nurse to join our home health team in Indiana. You will follow individualized care plans, monitor patient progress, and educate patients/caregivers on self-care while collaborating with RNs and physicians to ensure coordinated, high-quality care.

Requirements include an active LPN license, a nursing credential (certificate/diploma/associate), and at least 1 year of clinical experience.

Qualifications

• Licensed Vocational/Practical Nurse license required (LPN).
• Graduate of an accredited nursing program (certificate, diploma, or associate).
• Minimum 1 year of clinical care experience.

Responsibilities

• Follow patient care plans and physician orders.

Job Description:

As a Licensed Vocational / Practical Nurse you'll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.

• Monitor patient progress and report changes or safety concerns to the care team.

• Help patients and caregivers learn self-care techniques to support their health.

• Work with the RN Case Manager to update care plans as patient needs change.

• Document medication regimens and update medication profiles according to agency guidelines.

• Communicate with physicians, patients, families, and the care team to ensure coordinated care.

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.

• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

• 1 year of experience in a clinical care setting

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Experience in a hospice or home health environment is preferred
