Job summary

Avant Rehabilitation & Care Center in Trenton, NJ is seeking compassionate Licensed Practical Nurses (LPN) for all shifts. You will administer meds and treatments under the care of the Director of Nursing and Assistant Director of Nursing.

Responsibilities include following a designated care plan, wound care, and documenting resident observations while maintaining HIPAA privacy and residents’ rights. Qualified applicants live in or can work in Trenton, NJ.

Qualifications

• Valid LPN license in New Jersey.
• Experience in LTC/Skilled Nursing preferred.
• Available to work in Trenton, NJ.

As a Licensed Practical Nurse (LPN) you work closely alongside the Director of Nursing and the Assistant Director of Nursing to care for our residents. You will be administering medications and treatments in compliance with federal, state, local, and facility policies.

Shifts available

• 7am-3pm
• 3pm-11pm
• 11pm-7am

Duties & Responsibilities

• Order prescribed medications, supplies, and equipment as needed
• Follow a designated care plan in accordance with residents needs
• Perform assigned duties including wound care, treatments, and procedures
• Accurately document observations, interventions, and evaluations pertaining to resident
• Maintain confidentiality and rights of residents, family members, employees, and facility information in compliance with HIPAA policies

Qualifications & Requirements

• Valid LPN license in the State of New Jersey
• Experience in LTC/Skilled Nursing preferred
• Must be able to work in Trenton, NJ
• Compassionate working with the geriatric population
