Qualifications

Job Description

The LPN/LVN administers nursing care according to the plan of care to terminally ill patients admitted to the VITAS program, in accordance with the Nurse Practice Act in the State where the VITAS program is located.

Job Responsibilities
  1. Provides direct skilled bedside nursing care.
  2. Evaluates patient/family needs and assists in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
  3. Performs selected acts in the care of the terminally ill under the supervision of a registered nurse; such acts include the administration of treatments and medications.
  4. Evaluates effectiveness of treatments and monitors patient’s reactions.
  5. Reports significant findings or changes in the patient’s condition to the registered nurse, Team Manager or Physician. Documents reports in clinical notes.
  6. Maintains a clean and safe environment for the patient and family.
  7. Maintains accurate documentation including but not limited to clinical notes on interdisciplinary charts.
  8. Evaluates patient/family response to care.
  9. Utilizes a wide variety of supportive and technical nursing skills in carrying out specific medical orders for administration of medication, performing specific treatments, monitoring diagnostic, therapeutic, and supportive measures.
  10. Assists the interdisciplinary team in the designing and delivering of interdisciplinary plan of care.
  11. Establishes therapeutic relationships with patients/families and involves them in the plan of care to promote independence.
  12. Initiates and performs nursing protocols based on patient’s needs related to physical comfort and mental well-being, including positioning, mobilizing, elimination, skin and wound care, personal hygiene, and communication.
  13. Participates in patient/family teaching about illness, treatment, daily activities, self-care, medication, complication prevention, discharge, and follow-up.
  14. Plans and participates in professional growth activities, including in-service programs and community education.
  15. Supports education and research activities as assigned.
  16. Participates in on-call rotations as scheduled to meet patient and family needs.
  17. Willing to work flexible hours, including weekends and rotating shifts.
Job Requirements

Must have reliable transportation with required liability insurance and license.

Qualifications
  1. Graduate of an approved Practical/Vocational nursing program.
  2. Currently licensed to practice nursing in the state of the VITAS program.
  3. At least one year of satisfactory nursing experience, preferably in a hospital on an adult medical-surgical unit.
  4. Equivalent experience may be considered.
Additional Guidelines
  • Supervised by a Registered Nurse.
  • Reports to the applicable Team Manager.
  • Does NOT perform initial patient assessments.
