Job Description
The LPN/LVN administers nursing care according to the plan of care to terminally ill patients admitted to the VITAS program, in accordance with the Nurse Practice Act in the State where the VITAS program is located.
Responsibilities
- Provides direct skilled bedside nursing care.
- Evaluates patient/family needs and assists in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
- Performs selected acts in the care of the terminally ill under the supervision of a registered nurse; such acts include the administration of treatments and medications.
- Evaluates effectiveness of treatments and monitors patient’s reactions.
- Reports significant findings or changes in the patient’s condition to the registered nurse, Team Manager or Physician. Documents reports in clinical notes.
- Maintains a clean and safe environment for the patient and family.
- Maintains accurate documentation including clinical notes on interdisciplinary charts.
- Evaluates patient/family response to care.
- Utilizes a wide variety of supportive and technical nursing skills in carrying out specific medical orders for medication administration, treatments, and monitoring.
- Assists in designing and delivering the interdisciplinary plan of care.
- Establishes therapeutic relationships with patients and families to promote independence.
- Initiates nursing protocols based on patient needs, including positioning, mobilizing, wound care, hygiene, and communication.
- Participates in patient and family teaching about illness, treatment, and self-care.
- Participates in professional growth activities and research as assigned.
- Participates in on-call rotations and is willing to work flexible schedules, including weekends and shifts.
Job Requirements
- Must have reliable transportation, liability insurance, and a valid license.
Qualifications
- Graduate of an approved Practical/Vocational nursing program.
- Current nursing license in the state of practice.
- At least one year of nursing experience, preferably in a hospital setting.
- Equivalent experience may be considered.
