Job summary

Aviata Health Group is seeking a Licensed Practical Nurse (LPN) to join Aviata at The Harbor in Safety Harbor, FL. This on-site role provides direct patient care in a skilled nursing setting, assisting residents with daily living activities under the supervision of a registered nurse.

Responsibilities include implementing individualized treatment plans, supervising CNAs, overseeing Falls and Pressure Sore Programs, and conducting timely assessments.

Qualifications

• Must possess a current, unencumbered LPN license.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Demonstrated compassion and dedication to high-quality care.

Major Responsibilities

The Licensed Practical Nurse, LPN care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.

• Supervise CNAs and participate in their evaluation.

• Oversee Falls Program and Pressure Sore Program.

• Provide nursing services.

• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.

• Other duties as assigned.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.

• Experience in Skilled Nursing/Rehabilitation facilities preferred.

• You must be qualified, compassionate, and dedicated to a job well done.
