Job summary

Aviata Health Group in South Daytona, FL is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing team. You will provide daily patient care in a fast-paced facility, under supervision, and help implement care plans.

Job Summary:

Responsibilities:

• Provide direct patient care to residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse
• Assist in the implementation of individualized treatment plans for each assigned resident
• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation
• Oversee Falls Program and Pressure Sore Program
• Provide nursing services
• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment
• Other duties as assigned

Qualifications:

• Must possess a current, unencumbered, active state license to practice as an LPN
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• You must be qualified, compassionate, and dedicated to a job well done
