Job summary

Fairfield Memorial Hospital Association is seeking an LPN to join the Med/Surg team. Under RN supervision, you will provide direct patient care, administer meds, perform nursing procedures, and document in the EHR.

Responsibilities include IV management, patient education, and collaboration with the interdisciplinary team. Qualified candidates possess an Illinois LPN license and a current BLS certificate, with preferred Med/Surg or acute care experience.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Licensed Practical Nurse (LPN) license in the State of Illinois.
• Current BLS certification or willingness to obtain within six months of employment.
• IV Therapy Certification (preferred).
• Prior Med/Surg or acute care experience is preferred.

Responsibilities

• Provide direct patient care under supervision of an RN.
• Administer medications and treatments as ordered and document in the EHR.
• Perform nursing procedures to ensure patient comfort, safety, and well-being.
• Start and discontinue peripheral IVs and administer approved IV therapies.
• Monitor patients receiving blood products and report changes to the RN.
• Document assessments, interventions, treatments, and outcomes in the EHR.
• Participate in discharge planning and patient education using teach-back method.
• Communicate with physicians, nursing staff, patients, and families.
• Assist with patient transfers and mobility as needed.
• Support pain management and participate in quality improvement activities.

The LPN works under the direction and supervision of a Registered Nurse to provide safe, quality patient care across the lifespan. This position plays an important role in supporting patient recovery and well being through direct care, medication administration, patient education, and collaboration with the interdisciplinary healthcare team. The LPN is responsible for providing nursing care within the scope of practice established by the Illinois Nurse Practice Act and Fairfield Memorial Hospital policies and procedures.
