Job summary

Alternate Solutions Health Network seeks a licensed practical nurse to provide skilled home care under physician orders. You will follow RN plans, monitor patient conditions, and administer medications while educating patients and families on disease management.

The role requires strong communication, compassion, and teamwork; travel between assigned locations is expected, with on-call requirements and growth opportunities through ongoing training and a supportive team.

Qualifications

• Licensed Practical Nurse with current state license.
• Minimum of one year of experience as an LPN in an acute care setting required; two years preferred.
• Home care experience desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Ability to remain calm, patient, and accommodating.
• Knowledgeable on nursing best practices.
• Ability to identify situations and apply appropriate nursing judgments.
• Detail-oriented and observant.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Performs care following RN guidance and monitors patient conditions.
• Administers medication as prescribed by the physician.
• Counsels patient and family on disease/injury processes and management.
• Incorporates patient and family in development of plan of care.
• Maintains Medicare/Federal guidelines and completes clinical documentation.
• Participates in on-call requirements and case conferences.
• Maintains active LPN license in state of employment.

Major Areas Of Responsibility

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Performs care by following order sets and goals made by RN.
• Observes and monitors patient conditions.
• Notifies RNCM of any changes in patient condition.
• Administers medication as prescribed by the physician.
• Counsels patient and family on the disease/injury processes and how to manage.
• Incorporates patient and family in development of plan of care.
• Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
• Listens to patient and family members.
• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
• Documents all aspects of treatment, assessments, and patient education.
• Maintains active LPN license in state of employment.
• Communicates with scheduler any changes outside the normal.
• Participates in all on-call requirements and case conferences.
• Attends in-service trainings and mandatory agency meetings.
• Maintains a professional appearance as a representative of the company.
• Understands and practices agency policies and procedures.
• Other duties as assigned.

Essential Functions

• Drives to patient’s primary location per scheduled visit.
• Daily attendance at assigned visit locations.
• Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines.
• Completes and submits all required documentation within specified company requirements.
• Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.
• Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
