Job summary

Complete Care Management is hiring a Licensed Practical Nurse (Direct Care) in Delaware. We provide skilled nursing and therapy to recently discharged patients and long-term residents.

This role offers full or part-time schedules, PRN options with experience, and competitive benefits. Sign-on bonuses apply for FT and PT roles, with daily pay available as an option.

Qualifications

• Current Delaware LPN license or eligibility.
• Able to pass background check and drug screen.
• Professional & clinical with leadership qualities.

Responsibilities

• Administer medications (oral, topical, IV, injectable).
• Wound care and dressing management.
• Monitor health status and track changes.
• Perform catheter/ostomy care and specimen collection.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, and social workers.
• Educate residents and families on health status and care expectations.
• Maintain detailed health records and progress notes.
• Provide team oversight and delegate CNA tasks.

Here's What You'll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
