Job summary

Health Care Partners of South Carolina is seeking a Licensed Practical Nurse (LPN) to deliver high-quality patient care under physician or RN supervision. You will assess needs, perform nursing procedures, and educate patients while supporting a patient-centered care approach.

The role requires SC LPN licensure, BLS certification, and at least one year of clinical nursing experience, preferably in a primary care/clinic setting. Travel between clinics is expected.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current SC Licensed Practical Nurse (LPN) with unrestricted license.
• Current Basic Life Support (BLS) certification.
• Minimum of one year of clinical nursing experience preferred.
• Experience in FQHC/community health center preferred.
• Experience using EHR systems and Microsoft Office preferred.

Responsibilities

• Prepare patients for examination by obtaining and documenting medical histories, chief complaints, vital signs, and other clinical information in the EHR.
• Assess patient needs through observation, communication, physical assessment, triage, and diagnostic review; report findings to provider.
• Provide direct patient care including assessments, treatments, medications, immunizations, wound care, and other nursing procedures.
• Assist physicians and advanced practice providers with examinations, treatments, and minor procedures.
• Educate patients and families on medications, preventive care, chronic disease management, discharge instructions, and follow-up care.
• Coordinate laboratory testing, referrals, diagnostic procedures, and follow-up appointments.
• Conduct telephonetriage and follow-up calls within scope of practice and escalate when needed.
• Document all clinical activities in the EHR timely and accurately.
• Maintain exam rooms, equipment, and supplies, ensuring OSHA/infection control standards.
• Travel between Health Care Partners of South Carolina clinic locations as needed.

Position Summary

The Licensed Practical Nurse (LPN) provideshigh-quality nursing care under the direction of a physician, advanced practiceprovider, or Registered Nurse in accordance with the South Carolina NursePractice Act. The LPN is responsible for assessing patient needs, deliveringdirect patient care, assisting with examinations and procedures, administeringmedications and treatments, coordinating care, and educating patients and theirfamilies. This position plays a vital role in supporting the mission of HealthCare Partners of South Carolina by delivering compassionate, patient-centeredcare while promoting positive health outcomes for the communities we serve.

EssentialDuties and Responsibilities

• · Prepare patientsfor examination by obtaining and documenting medical histories, chiefcomplaints, vital signs, and other clinical information in the Electronic Health Record (EHR).
• · Assess patientneeds through observation, communication, physical assessment, telephonetriage, and review of diagnostic information, reporting significant findings tothe provider or Registered Nurse.
• · Provide directpatient care, including assessments, treatments, medication administration,immunizations, injections, wound care, dressing changes, and other nursingprocedures within the LPN scope of practice.
• · Perform clinicalprocedures including specimen collection, electrocardiograms (EKGs),point-of-care testing, inhalation treatments, and other delegated duties asappropriate.
• · Assist physiciansand advanced practice providers with examinations, treatments, and minorprocedures.
• · Educate patientsand families regarding medications, preventive care, chronic diseasemanagement, treatment plans, discharge instructions, and follow-up care.
• · Coordinatelaboratory testing, referrals, diagnostic procedures, and follow-upappointments as directed by the provider.
• · Conduct telephonetriage and follow-up calls, providing appropriate guidance within the scope ofpractice and escalating concerns to the provider when necessary.
• · Accuratelydocument patient assessments, treatments, communications, and clinicalactivities in the Electronic Health Record (EHR) in a timely manner.
• · Support qualityimprovement initiatives by participating in chart audits, quality assuranceactivities, and organizational initiatives, including HRSA Uniform Data System(UDS) measures.
• · Maintainexamination rooms, medical equipment, and clinical supplies while ensuringcompliance with OSHA, infection prevention, and safety standards.
• · Monitor medicalsupplies, medications, and vaccines for inventory levels and expiration dates,and notify appropriate personnel of equipment or supply needs.
• · Maintain patientconfidentiality and protect health information in accordance with HIPAA andorganizational policies.
• · Respondappropriately to medical emergencies following established clinical protocols.
• · Collaborate withproviders, nursing staff, and interdisciplinary care teams to ensurecoordinated, patient-centered care.
• · Participate inrequired trainings, continuing education, staff meetings, and organizationalquality improvement initiatives.
• · Travel betweenHealth Care Partners of South Carolina clinic locations as needed.

Education &Experience

• · Graduate of anaccredited Practical Nursing program required.
• · Current,unrestricted South Carolina Licensed Practical Nurse (LPN) license required.
• · Current BasicLife Support (BLS) certification through the American Heart Association orAmerican Red Cross required.
• · Minimum of one(1) year of clinical nursing experience in a physician practice, outpatientclinic, community health center, or similar healthcare setting preferred.
• · Experienceworking in a Federally Qualified Health Center (FQHC) or community healthcenter preferred.
• · Experience usingElectronic Health Record (EHR) systems and Microsoft Office preferred.
